Which chords are you trying to practice? The first chords I learned were G, Em, C, and D. For a long time I couldn't grasp the idea of how to switch from D to G but just like Old Tele man said, it just takes practice.
It will get easier when you memorize the exact positioning of your fingers for each chord and just keep playing.
You can also try learning scales so that you can exercise your fingers more. I think doing that will REALLY help you in the long run.

Hey, what I found to be best was to just use my left hand and go between two chords I was having trouble with as quickly as I felt all of the strings make a solid connection with the fret board. I hated the sound of strumming though with my weak fingers so I only strummed when I was worried I was doing the left hand wrong. Eventually when my hand got stronger I was able to sound the chord as if it were a hammer-on / pull off which i found to be way more entertaining than just strumming at the quick pace I had reached.

Im a beginner guitarist myself and have just recently started getting good at my first chord sequence, Am-Dm-Am-E. The way I was told to work on it was to use a metronome. you can get one from a local music shop for undr $15 and it really helps. Set the metronome to the lowest speed then strum each chord once then switch. once you can do this at about 120 bpm which is pretty average pace then you can start adding strumming patterns. It does take alot of practice but I've found using a metronome for 20min each day and just grinding throught the horrible sounding moments in the begining yeilds cleaner chord transistions faster than practicing without a metronome.
Hope That Helps!!!
